Exploratory Randomized Double-Blind Placebo-Controlled Trial of Botulinum Therapy on Grasp Release After Stroke (PrOMBiS)

본 연구는 뇌졸중 이후 grasp release 에 대한 OnabotulinumtoxinA 의 효과를 규명한 연구이다.

뇌졸중 이후 최소 1개월이 지난 시점의 28명의 환자를 enroll 하였고, enroll 된 환자들은 무작위 배정으로 treatment group (OnabotulinumtoxinA injections 1회 시행 이후 4주간 10세션의 standardized upper-limb physioltherapy), placebo group (placebo injection 1회 시행 이후 treatment group 과 동일한 재활치료) 로 배정하였다 (Fig.1).

각 그룹간의 demographics 에는 유의미한 차이가 없음을 확인하였다 (Table 1).

OnabotulinumtoxinA 의 용량, 부위는 portable hand-held device (Clavis; Medtronic, Minneapolis, MN) 을 활용하여 standard neurophysiological technique 를 통해 평가하였고, 개인화된 치료가 이루어졌다 (Table 2). OnabotulinumtoxinA 는 saline 2mL 에 100units 을 추가하여 희석하였다.

Primary outcome 은 injection 이후 5주 시점에서 functionally relevant standardized task 를 수행하는 동안 grasp 를 release 하는데 걸리는 시간(초) 으로 하였다. Secondary outcome 으로 wrist 와 finger 의 spasticity, customized servomotor 를 활용한 상지 근력, modified Ashworth Scale, Action Research Arm Test (ARAT), Nine Hole Peg Test 등을 확인하였다.

두 그룹간 grasp release time 에는 유의미한 차이가 없었고 (placebo median = 3.0 s, treatment median = 2.0 s; t(24) = 1.20; P = .24; treatment effect = −0.44, 95% CI = −1.19 to 0.31) (Fig.2.), secondary outcome 에서도 통계적으로 유의미한 차이는 확인할 수 없었다.
