Vascular cognitive impairment 동물모델에서 neuroimaging biomarker를 이용한 brain의 structural connectivity 변화 예측

Vascular cognitive impairment의 경우 microbleeds, lacunar infarctions, diffusion white matter hyperintensities 등 다양한 영상학적 양상을 보인다. Hypoperfused mice에서 cognitive deficits, 특히 spatial working memory impairment가 보인다고 보고된 바 있으며, underlying pathologic change는 degraded myelin basic protein의 accumulation, white matter structure내의 myelin-associated glycoprotein distribution의 변화, fluoromyelin staining의 소실, matrix-metalloproteinase-9의 증가 등 다양하게 나타났다. 본 논문은 이러한 pathologic change를 neuroimaging으로 확인하고자 한 내용이다.

연구자들은 이전에 hypoperfused mice에서 diffusion tensor image (DTI)의 fractional anisotropy (FA)의 감소를 관찰하는데 실패했으나, 다른 parameter에서는 의미있는 변화가 관찰되어, 좀더 severe하게 cognitive deficit을 유도하여, DTI data와 cognitive decline의 연관성을 보고자 했다.

Hypoperfusion model은 160 µm 직경의 microcoil을 이용해서 만들었고 한쪽 carotid artery 시술 24시간 후 반대쪽 carotid artery 시술했다. 시술 전, 시술 24시간후, 1주후, 4주후 arterial spin labeling으로 cerebral blood flow (CBF)를 측정했고, DTI, MRspectroscopy, MRA는 시술 5-7주 사이에 시행했다. 시술 1주전, 그리고 시술 4-5주 사이에 novel object recognition test를 했고, 6주째에 Morris water maze로 1주간 training, 8일째 probe trial을 시행했다.

Hypoperfusion 유도 24시간째 CBF는 반 이상 감소했으며, 시술 1주내에 증가하기 시작하여 지속적으로 호전되었지만, 4주째에도 여전히 감소되어 있었다. Hypoperfusion이 심해지면 spatial learning ability가 떨어졌다. Histology 결과 severe white matter change를 확인할 수는 없었지만, diffusion tensor image data에서 hypoperfused brain network의 substantial reorganization이 확인되었다. DTI data 중 clustering coefficient가 water maze performance와 관련이 있었고, global, local efficiency와 degrees의 세 parameter가 cognitive deficit을 예측하는데 효과적이었다.

논문 결과, 저자들은, Mouse brain은 갑자기 perfusion을 떨어뜨렸을 때 이를 compensation함에도 불구하고 brain network의 변화가 확실히 나타나는 것으로 보여, 이를 outcome예측의 neuroimaging biomarker로 사용할 수 있을 것으로 생각된다고 결론을 내렸다.
