Structural Connectivity Affecting Aspiration After Stroke

Introduction
삼킴 장애(Dysphagia)는 뇌졸중 후 흔히 발생하는 증상으로 흡인성 폐렴 등의 심각한 합병증을 유발할 수 있기 때문에, 모든 뇌졸중 환자에서 조기 진단 및 적극적인 치료가 필요한 증상이다. 뇌졸중 후 발생하는 삼킴 장애의 원인 뇌 병변에 대해서는 여러 연구가 보고되고 있지만 아직까지 명확한 뇌 병변은 알려져 있지 않다.
Objectives
본 논문에서는 structural connectivity 분석을 통해 뇌졸중 후 삼킴 장애가 일어나는 뇌 신경망 특성에 대해 알아보고자 한다.
Methods
대상자: 40명의 뇌졸중 환자
Inclusion criteria
First-ever stroke
Supratentorial stroke
Mini-Mental Status Examination score > 18
Oral feeding without a Levin tube or percutaneous enteral tube feeding
MRI and Video Fluoroscopic Swallowing Study (VFSS) performed within a 1-week interval
> 1 month after stroke onset
Exclusion criterion
Any coexisting neurological or psychiatric disease
Aspiration assessment: VFSS
Aspiration group; 22 patients
Normal group; 18 patients).
Structural connectivity assessment: Voxel-based lesion–symptom mapping with diffusion tensor imaging (DTI)
통계 분석: Comparison of characteristics between the two groups
Results
임상적 특성: 양군에서 임상적 특성에 차이를 보이지 않았음 (Table 1).

Structural Connectivity: 삼킴 장애가 있는 뇌졸중 환자(Aspiration group)에서 Right anterior limb and genu of the internal capsule의 Fractional anisotropy (FA), mode of anisotropy (MO)의 증가 소견이 관찰되었음 (Fig. 1).

Conclusions
본 연구 결과를 통해 우측 대뇌 반구의 internal capsule을 지나는 tract의 degeneration이 뇌졸중 후 삼킴 장애와 높은 연관성을 보이는 것을 알 수 있었다. 향후 보다 많은 뇌졸중 환자를 대상으로 white matter의 degeneration 분석을 통한 삼킴 장애 발생 원인에 대한 추가 연구가 필요할 것으로 판단된다.
