A Genomic Risk Score Identifies Individuals at High Risk for Intracerebral Hemorrhage

Introduction

뇌내출혈(Intracerebral hemorrhage, ICH)는 유전적 원인이 높은 질환으로 알려져 있다. Genome-wide association studies (GWAS)를 통해 이러한 유전적 원인에 대한 분석이 이루어짐에 따라 보다 정확한 유전적 위험성에 대한 자료를 제시할 수 있게 되었다.

Objectives

본 논문에서는 ICH에 대한 genomic risk score (GRS)를 개발하고 표준 임상 위험 요소와 비교하여 예측력을 알아보고자 한다.

Methods

l  Data source: genotype and phenotype data (1,861 ICH cases and 1,722 ICH-free controls)

l  Validation dataset (842 cases and 796 controls)

–       ICH and its predictive performance in addition to clinical risk factors

l  Tested with risk of incident ICH in the population-based UK Biobank cohort

–       486,784 individuals

–       1,526 events

–       median follow-up 11.3 years

Results

l  Construction of a MetaGRS for ICH in the Training Dataset

A. Individual genomic risk scores (GRS) were derived for intracerebral hemorrhage (ICH)-related traits from publicly available summary statistics.

B. The GRS were optimized in the combined training dataset of GOCHA and EUR/ISGC.

l  Meta-genomic risk score (metaGRS)

–       The metaGRS was significantly associated with 31% higher odds for ICH (95% CI, 1.16–1.48) in age-, sex- and clinical risk factor-adjusted models.

–       The metaGRS identified individuals with almost 5-fold higher odds for ICH in the top score percentile (odds ratio, 4.83 [95% CI, 1.56–21.2]).

–       Predictive models for ICH incorporating the metaGRS in addition to clinical predictors showed superior performance compared to the clinical risk factors alone (c-index, 0.695 versus 0.686).

Conclusions

본 연구 결과를 통해 ICH의 위험 정도를 유전자 특성의 정량화로 계산된 metaGRS로 평가할 수 있음을 확인하였다. 이를 통해 장기간 항혈전제 사용을 요하는 ICH 발생 위험이 높은 환자에서 약물 종류 및 사용 기간 등의 임상적 결정에 유전자 특성을 고려할 수 있을 것으로 기대된다.
