Tirofiban for Stroke without Large or Medium-Sized Vessel Occlusion

Introduction

지금까지 여러 관찰연구들이 Tirofiban (fast-acting, highly selective, low-molecular weight nonpeptide glycoprotein IIb/IIIa receptor inhibitor)가 특정 급성 뇌경색 환자에게 효과적이라는 결과를 보여주었다.

 

Objectives

RESCUE BT2 trial은 무작위 대조연구로 Large or medium sized intracranial vessel occlusion이 없는 급성 뇌경색 환자에서 Tirofiban의 잠재적인 효과를 확인하고자 하였다.

 

 

Methods

ž   Design

-       multicenter, doble-blind, double-dummy, randomized clinical trial로 중국내 시행

-       아스피린과 비교하여 Tirofiban의 Efficacy와 Safety를 확인

-       Large or medium sized intracranial vessel occlusion이 없는 Recent-onset ischemic stroke

ž   Intervention

-       4가지 가운데 한가지 상태를 가진 환자를 대상

①      Last normal time 기준 24시간 이내 이면서 혈전용해 또는 혈전제거치료가 불가능

②      24~96시간 이내 환자이면서 증상 악화

③      혈전용해치료후 조기신경학적악화 (early neurological deterioration)

④      혈전용해치료후 4~24시간 동안 증상 호전이 없는 환자

-       총 1177명의 환자 등록

①    48시간까지 : Aspirin 100mg : IV Tirofiban

②    48시간이후 ~ 90일 : Aspirin 100mg

-       Primary outcome : 90일후 Excellent outcome (Modified Rankin Score 0 또는 1점)

 

 

Results

ž   Efficacy: 3개월후 Excellent outcome (mRS 0 or 1) 비율은 Tirofiban군에서 Aspirin군에 비하여 유의하게 높았다 (29.1% vs. 22.2%, adjusted risk ratio, 1.26; 95% CI, 1.04 to 1.53, p=0.02)

ž   Safety: 3개월후 사망률은 양군에서 비슷하게 나타났다. 증상뇌출혈 (symptomatic intracranial hemorrhage)은 Tirofiban 군에서 1%(6명/606명) 발생하였고 Aspirin 군에서는 발생하지 않았다.

 

 

 

 

Limitation

ž   대부분의 환자가 atherosclerotic event로 cardioembolic stroke에 적용이 제한이 있다.

ž   중국인만을 대상으로 하였고, 혈전 용해제를 사용한 군이 적었다.

 

 

Conclusions

Large or medium-sized intracranial vessel occlusion이 없는 recent-onset ischemic stroke에서 정맥내 Tirofiban 사용은 aspirin 대비하여 90일의 Excellent outcome을 가질 가능성을 높여준다.

 

 

Comment

급성 뇌경색 치료에서 Tirofiban 사용의 근거를 마련해준 첫 연구결과로 향후 Tirofiban의 임상적 사용과 연구가 추가적으로 이루어 질것으로 예상된다.
